What CYFRA 21-1 is and why it is measured

CYFRA 21-1 is a tumour marker — a protein fragment that comes from cytokeratin 19, a structural protein found in large amounts inside many epithelial cells. When such cells break down or multiply faster than usual, fragments of cytokeratin 19 get into the blood, and so the concentration of CYFRA 21-1 can rise. According to MedlinePlus, tumour marker tests in general are not intended to diagnose cancer in a healthy person — they are most often done when an oncological disease is already known. They help to assess the stage of the disease, to anticipate its likely course, to choose the treatment approach, to follow whether the treatment is working, and to watch whether the disease has come back after treatment. Like other similar markers, the amount of CYFRA 21-1 can rise not only in cancer. That is why the result of a single test is always assessed together with your doctor, the history of the illness, the clinical picture and other tests, rather than on its own and not as a diagnosis in its own right.

When CYFRA 21-1 is tested

  • When an oncological disease is already known — to obtain additional information about the stage and the course of the disease
  • During cancer treatment (chemotherapy, surgery, radiotherapy) — following whether the body is responding to the treatment being given
  • After treatment has been completed — checking periodically whether the disease has come back
  • When a doctor wants to compare a new result with earlier tests, assessing how the value changes over time
  • Together with other tests and imaging methods, when a broader picture of the oncological condition is needed

How to prepare

  • An ordinary blood test most often needs no special preparation
  • If you have an acute illness or inflammation at the time, it is worth telling your doctor — it can affect how the result is interpreted
  • It makes sense to discuss the result with a doctor who knows the history of the illness and the earlier tests

What counts as a normal CYFRA 21-1

No reliable, publicly available source from an international health institution giving a specific numeric reference interval was found for this marker, so we do not present one here. The exact laboratory reference interval, which depends on the method used, is always given on the test report.

Who assesses the result

The result is always discussed by a doctor, most often comparing it with the same patient's earlier tests and the known history of the illness, because the value of a single result without that context gives little information about health. According to MedlinePlus, tumour marker tests in general are not intended for screening a healthy person — they are used purposefully, when an oncological diagnosis is already present, to follow the course of the disease and the treatment.

Frequently asked questions

›What is the normal range for CYFRA 21-1?

No reliable, publicly available source from an international health institution giving a specific numeric reference interval was found for this marker, so we do not present one here. The exact laboratory reference interval, which depends on the method used, is always given on the test report.

›Is CYFRA 21-1 used to diagnose cancer?

According to MedlinePlus, tumour marker tests in general are not used as a screening or diagnostic tool for a healthy person. They are most often done when an oncological disease is already known — to follow the response to treatment and to watch for a possible recurrence after treatment.

›What is cytokeratin 19, which CYFRA 21-1 comes from?

Cytokeratin 19 is a structural protein found in large amounts inside many epithelial cells. When such cells break down or multiply faster than usual, its fragments get into the blood — and that is what the CYFRA 21-1 test measures.

›Does a CYFRA 21-1 test need any special preparation?

An ordinary blood test most often needs no special preparation. It is worth mentioning to your doctor if you have an acute illness or inflammation at the time, because that can affect how the result is interpreted.

›Who assesses the result of a CYFRA 21-1 test?

The result is always discussed by a doctor, most often comparing it with the same patient's earlier tests and the known history of the illness, because the value of a single result without that context gives little information about health.

›Is CYFRA 21-1 suitable for screening a healthy person?

No. According to MedlinePlus, tumour marker tests in general are not intended for screening a healthy person — they are used purposefully, when an oncological diagnosis is already present, to follow the course of the disease and the treatment.
